Faculty Disclosure
It is the policy of the University of South Florida College of Medicine and the American Telemedicine Association to ensure fair balance, objectivity, and scientific rigor in all presentations. Faculty are expected to disclose any significant relationships with commercial supporters of the annual meeting and/or with manufacturers of any commercial products or services discussed during the educational presentation. Faculty must also disclose any off-label use of products that will be discussed during their presentation. Faculty disclosures are provided for review to all attendees.

Physicians

Registered Dieticians
The University of South Florida, College of Public Health, CO001 is a Continuing Professional Education (CPE) Accredited Provider with the Commission on Dietetic Registration (CDR) from 2/22/07 to 2/21/10. Registered dietitians (RDs) and dietetic technicians, registered (DTRs) will receive up to 19.75 continuing professional education units (CPEUs) for completion of this program/materials. Continuing Professional Education Provider Accreditation does not constitute endorsement by CDR of a provider, program or materials.

Florida Licensed Clinical Social Workers, Licensed Marriage and Family Therapists
Licensed Mental Health Counselors
The University of South Florida College of Public Health is an approved provider (BAP#433 – Exp. 3/31/09) of continuing education credits for clinical social work, marriage and family therapy, and mental health counseling.
